What Is Blepharoplasty?

Blepharoplasty—commonly called an eyelid lift or eye lift—is a surgical procedure that rejuvenates the appearance of the upper eyelids, lower eyelids, or both. It removes excess skin, eliminates fatty deposits that cause puffiness, and tightens underlying muscles to create a more alert, youthful appearance.

The procedure can be performed for cosmetic reasons, to look more rested and rejuvenated, or for functional reasons when drooping upper eyelids actually obstruct vision. Either way, the goal is natural-looking results that enhance your eyes without looking "done."

Common Eye Area Concerns

These are the signs of aging that bring patients to our office—and that blepharoplasty can dramatically improve.

Hooded Upper Eyelids

Excess skin that folds over the natural crease, making eyes look heavy and tired

Under-Eye Bags

Puffy fat deposits beneath the eyes that create a tired, aged appearance

Drooping Eyelids

Sagging skin that can even obstruct peripheral vision in severe cases

Looking Tired

Constantly being asked "Are you tired?" despite feeling well-rested

Excess Lower Eyelid Skin

Wrinkled, loose skin under the eyes creating fine lines and crepiness

Difficulty with Makeup

Eyeshadow disappears into folds, eyeliner looks uneven or smudged

Types of Eyelid Surgery

Blepharoplasty can be customized to address your specific concerns. Many patients benefit from treating both upper and lower eyelids together.

Upper Blepharoplasty

Upper Eyelid Surgery

Removes excess skin and fat from the upper eyelids to eliminate hooding and reveal the natural eyelid crease.

  • Removes hooded, heavy eyelids
  • Opens up the eyes
  • Scar hidden in natural crease
  • Can improve obstructed vision

Lower Blepharoplasty

Under-Eye Surgery

Removes or repositions fat, tightens skin, and smooths the under-eye area to eliminate bags and puffiness.

  • Eliminates under-eye bags
  • Smooths wrinkled skin
  • Can use hidden incision inside lid
  • Reduces dark circles

Why Choose Dr. Neavin for Your Eyelid Surgery

"The eyes are the centerpiece of the face. Every millimeter matters. My goal is for you to look refreshed and like yourself—just more rested."

Eyelid surgery requires an exceptionally delicate touch. The skin around the eyes is the thinnest on the body, and the margin for error is measured in millimeters. Remove too much skin, and the eyes look pulled or unnatural. Remove too little, and you won't see the improvement you hoped for.

With over 20 years of experience in facial plastic surgery, I approach every blepharoplasty with a conservative philosophy: enhance, don't overcorrect. I'm also careful to evaluate whether your concerns are truly from the eyelids or from brow descent—a distinction that changes the surgical plan entirely.

All eyelid surgeries are performed in our accredited surgical facility with meticulous attention to safety, comfort, and natural-looking results.

The Blepharoplasty Procedure

Here's what to expect from your eyelid surgery journey.

1

Consultation

We carefully examine your eyes, eyelids, and brow position to determine the best approach. Sometimes what appears to be upper eyelid sagging is actually brow descent—we'll identify the true cause and recommend the right solution.

2

Surgery Day

Blepharoplasty is typically performed under local anesthesia with sedation, taking 1-2 hours depending on whether we're treating upper, lower, or both eyelids. Incisions are placed in natural creases where they become virtually invisible.

3

Immediate Recovery

You'll go home the same day with cold compresses for the eyes. Expect swelling and some bruising that peaks around day 2-3. Lubricating eye drops help with any temporary dryness.

4

Results

Sutures are removed around day 5-7. Most patients feel comfortable going out in public by week 2, with makeup covering any residual bruising. Final results are visible at 3 months and last 10-15+ years.

Recovery Timeline

Blepharoplasty has one of the easiest recoveries of any facial procedure.

Day 1-3

Peak Swelling

Rest with head elevated. Cold compresses. Eyes may feel tight.

Day 5-7

Sutures Out

Bruising starts to fade. Most swelling resolved. Stitches removed.

Week 2

Back to Normal

Most return to work and social activities. Makeup can cover residual bruising.

Month 3

Final Results

All swelling gone. Scars faded. Final, natural results visible.

Often Combined With

For comprehensive facial rejuvenation, blepharoplasty is frequently paired with these complementary procedures.

Brow Lift

If sagging brows contribute to heavy upper eyelids, a brow lift may be recommended alongside or instead of upper blepharoplasty.

Facelift

Addresses the lower face and neck to create harmony between your refreshed eyes and the rest of your face.

Botox & Fillers

Non-surgical options to soften crow's feet, lift brows subtly, and fill hollow areas around the eyes.

Laser Resurfacing

Improves skin texture, fine lines, and pigmentation around the eyes for a more complete rejuvenation.

Am I a Good Candidate?

Most adults with eye area concerns are candidates for blepharoplasty. Here's how to know if it's right for you.

Ideal Candidates

  • Drooping or hooded upper eyelids
  • Puffy bags under the eyes
  • Excess skin obscuring the natural eyelid crease
  • Good overall health
  • Realistic expectations
  • Non-smoker (or willing to quit temporarily)

Important Considerations

  • Dry eye syndrome (may need pre-treatment)
  • Thyroid eye conditions
  • Glaucoma or other eye diseases
  • Primary concern is brow drooping (may need brow lift)
  • Dark circles without puffiness (may need filler)
  • Unrealistic expectations

Blepharoplasty FAQ

How much does eyelid surgery cost?
In Beverly Hills, upper blepharoplasty typically ranges from $4,000 to $6,000, lower blepharoplasty from $5,000 to $7,000, and combined upper and lower surgery from $8,000 to $12,000. This includes surgeon fees, anesthesia, and facility costs. I'll provide a detailed quote after your consultation.
How long do blepharoplasty results last?
Blepharoplasty results are long-lasting—typically 10-15 years or more. Many patients never need a second procedure. While your eyes will continue to age, the improvements from surgery are permanent, meaning you'll always look younger than if you hadn't had the procedure.
Will there be visible scars?
Scars from blepharoplasty are extremely well-hidden. Upper eyelid incisions are placed in the natural crease, making them virtually invisible once healed. Lower eyelid incisions are placed just below the lash line or inside the eyelid (transconjunctival approach), leaving no visible scar at all.
Is blepharoplasty painful?
Most patients report minimal pain—more like tightness and mild discomfort than actual pain. The procedure is performed under local anesthesia with sedation, so you feel nothing during surgery. Afterward, over-the-counter pain medication is usually sufficient. Eyes may feel dry and slightly irritated for a few days.
When can I wear makeup after surgery?
You can typically resume wearing eye makeup about 10-14 days after surgery, once the incisions have fully closed and any sutures have been removed. Makeup can actually help camouflage any residual bruising or swelling during the later stages of recovery.
Can insurance cover eyelid surgery?
If drooping upper eyelids significantly obstruct your vision, the functional component of upper blepharoplasty may be covered by insurance. This requires documentation including visual field testing that demonstrates the obstruction. Cosmetic eyelid surgery is not covered. Our staff can help you understand your coverage options.
What's the difference between blepharoplasty and a brow lift?
Blepharoplasty removes excess skin from the eyelids themselves. A brow lift raises sagging eyebrows. Sometimes what appears to be heavy upper eyelids is actually caused by drooping brows pushing skin down onto the lids. During your consultation, I'll examine both areas and recommend the procedure (or combination) that will give you the best result.
What age is best for eyelid surgery?
There's no "best" age—it depends on when aging changes bother you enough to address them. Most patients are between 35 and 65, but I've performed blepharoplasty on patients in their 20s with genetic eye bags and patients in their 70s who wanted to look more alert. The right time is when you're ready.
