What Is Breast Reduction?

Breast reduction, medically known as reduction mammaplasty, is a surgical procedure that removes excess breast tissue, fat, and skin to achieve a breast size more proportionate to your body. Unlike a breast lift (which primarily repositions), reduction actually decreases volume while also lifting and reshaping.

The result is lighter, smaller breasts that sit higher on the chest—relieving strain on your neck, shoulders, and back while creating a more balanced silhouette. For many women, this procedure eliminates pain they've lived with for years.

Benefits of Breast Reduction Surgery

Breast reduction addresses both physical discomfort and aesthetic concerns, often delivering immediate relief that patients describe as "life-changing."

Why Choose Dr. Neavin for Breast Reduction

"Breast reduction isn't just about removing tissue—it's about creating a breast that looks natural, sits well, and holds its shape over time. That takes surgical precision and an aesthetic eye."

With over 20 years of experience performing breast surgery in Beverly Hills, I've developed techniques that prioritize both function and aesthetics. My approach to breast reduction focuses on three key principles: adequate reduction to relieve symptoms, beautiful breast shape with natural projection, and well-hidden scars that fade over time.

As an internationally recognized lecturer on fat transfer techniques, I bring a unique perspective to breast reduction. In some cases, I use fat grafting to refine contours or correct asymmetry—achieving results that would be difficult with tissue removal alone. This dual expertise allows me to customize each procedure to the patient's specific anatomy and goals.

I perform all breast reduction surgeries at Cedars-Sinai Medical Center, one of the nation's top hospitals, where patients have access to the highest standards of safety and care. Every patient receives my direct attention from consultation through recovery.

Breast Reduction Incision Techniques

The incision pattern depends on how much tissue needs to be removed and your breast anatomy. During consultation, I'll recommend the technique that achieves your goals with the least visible scarring.

Donut (Periareolar)

Minor Reductions

Incision only around the areola. Best for small reductions with minimal excess skin. Results in the least visible scarring.

Lollipop (Vertical)

Moderate Reductions

Incision around areola plus vertical line to breast crease. Ideal for moderate reductions while avoiding horizontal scar.

Anchor (Wise Pattern)

Significant Reductions

Full anchor-shaped incision for maximum tissue removal and reshaping. Necessary for large reductions; scars are well-concealed.

What to Expect During Breast Reduction Surgery

Your Consultation

During your consultation, we'll discuss your symptoms, goals, and what size you'd like to be after surgery. I'll examine your breasts, assess tissue quality and nipple position, and explain which technique will work best for your situation. We'll look at before-and-after photos of patients with similar starting points so you can see realistic outcomes. If you're considering insurance coverage, we'll discuss documentation requirements at this time.

The Surgical Procedure

Breast reduction is performed under general anesthesia and typically takes 3-4 hours depending on the amount of tissue being removed. I remove excess breast tissue, fat, and skin, then reshape the remaining tissue into a smaller, lifted breast. The nipple and areola are repositioned higher on the breast mound—in most cases, they remain attached to their blood supply (a "pedicle") to preserve sensation and potential breastfeeding ability. If your areolas are large, I can reduce their diameter at the same time. The incisions are closed in layers for optimal healing and minimal scarring.

Anesthesia and Safety

I perform breast reduction surgery at Cedars-Sinai Medical Center's outpatient surgical facility, where you'll have access to board-certified anesthesiologists and the highest safety standards. Most patients go home the same day, though overnight observation can be arranged if preferred. You'll need someone to drive you home and stay with you the first night.

Breast Reduction Recovery

Most patients are surprised by how manageable recovery is—and how quickly they notice relief from their previous symptoms.

Insurance Coverage for Breast Reduction

Unlike most cosmetic procedures, breast reduction may be covered by insurance when deemed medically necessary. Coverage typically requires documentation of symptoms that have persisted despite conservative treatment.

Common insurance requirements include:

  • Documentation of chronic neck, shoulder, or back pain attributed to breast size
  • Evidence of skin rashes or infections under the breasts
  • Records of conservative treatments tried (physical therapy, prescription bras, medications)
  • Minimum grams of tissue to be removed (varies by insurer and patient BMI)

My staff is experienced in working with insurance companies and can help you understand your coverage. Even if insurance doesn't cover the full cost, partial reimbursement is often possible.

Breast Reduction FAQ

How much smaller can I go?
There's no universal limit—the goal is a size proportionate to your frame that relieves symptoms. During consultation, I'll discuss what's realistic based on your anatomy. Most patients drop 1-3 cup sizes, though some go smaller. I use your input along with measurements to determine the right amount of tissue to remove.
Will I be able to breastfeed after reduction?
Many women can breastfeed after reduction, though some experience reduced milk supply. The technique I use preserves the connection between the nipple and breast tissue whenever possible. If future breastfeeding is important to you, let me know—I can discuss how to maximize your chances while still achieving meaningful reduction.
Will I lose nipple sensation?
Temporary changes in nipple sensation are common and typically resolve within a few months. Permanent loss of sensation is possible but uncommon with modern techniques. If you have very large breasts requiring a "free nipple graft" technique, sensation is more likely to be affected—I'll discuss this if it applies to your situation.
What about scarring?
Breast reduction does leave permanent scars, but they're designed to be easily concealed. Scars around the areola blend with the natural color transition. Vertical and horizontal scars fall in areas covered by bras and swimsuits. Most scars fade significantly over 12-18 months. I provide scar care instructions to optimize healing.
How much does breast reduction cost?
Cost varies based on the extent of reduction needed and whether insurance covers any portion. For patients paying out-of-pocket, I'll provide a detailed quote after your consultation that includes surgeon fees, facility costs, and anesthesia. Financing options are available.
What's the difference between breast reduction and a breast lift?
A breast lift (mastopexy) repositions sagging breasts without significantly changing their size. Breast reduction removes tissue to decrease volume while also lifting. If your primary concern is sagging rather than size, a lift may be more appropriate. If size is causing symptoms, reduction addresses both concerns at once.
When can I exercise after surgery?
Light walking is encouraged immediately. Most patients return to lower body exercise at 3-4 weeks and resume full activity including upper body workouts at 6 weeks. I'll clear you for specific activities based on your healing progress.
How long do results last?
Breast reduction results are long-lasting. Your breasts will still change with age, weight fluctuations, and gravity, but you won't return to your previous size. Maintaining a stable weight helps preserve your results. Pregnancy can affect breast size, so some patients prefer to wait until they're done having children.
